4.  If self employed or commissioned last (2) years signed tax returns

5. Last (2) months bank statements for all declared assets.  Checking & savings accounts and or stock accounts

6.  Last statement for 401K/IRA's

7.  Divorce decree if applicable

8.  Bankruptcy papers if applicable (discharge papers also)

9.  $350 check for appraisal.
